Arthroscopy, also called key-hole joint surgery is one of the latest and advanced treatment modalities to treat joint injuries. In this procedure, using fine telescopes, the inside of the joint is visualized and surgical procedures are carried out using specialized instruments. Arthroscopic procedures for treating knee ligament tears and meniscus tears are the gold standard of treatment. In addition shoulder problems like dislocations, rotator cuff tears are treated arthroscopically . Arthroscopy has a significant role in the management of an injured athlete and allows the athlete to return to the previous level of activity.

A common indication for arthroscopy is reconstruction of torn ligaments of the knee namely the anterior cruciate ligament .The anterior cruciate ligament (ACL) is one of the most commonly injured ligaments of the knee. In general, the incidence of ACL injury is higher in people who participate in high-risk sports, such as basketball, football, skiing, and soccer. However , ACL injuries are very commonly seen with road traffic accidents also . As sports have become an increasingly important part of day-to-day life over the past few decades, the number of ACL injuries has steadily increased. This injury has received a great deal of attention from orthopedic surgeons over the past 15 years, and is one of the most successful surgeries in the field of Arthroscopic surgery today.

ANTERIOR CRUCIATE LIGAMENT ( ACL )

Anterior cruciate ligament is one of the commonest knee injuries in sports and day-to-day life. It is estimated that around 1,00,000 to 2,00,000 ACL injuries occur per year in United States and roughly around 70,000 ACL reconstructions are performed. Patients with ACL insufficiency can walk in a straight line. But when they attempt to change the direction suddenly while walking or running they may feel the knee joint giving away. Additional symptoms like Pain and repeated joint swellings may be present due to additional injuries to meniscus and cartilage.

ACL tears do not have the capacity to heal by themselves. This is because of the poor blood supply and the presence of joint fluid. Similarly the torn edges of the ligament cannot be sutured together, as this procedure has very high failure rates and has been given up. The ideal way to get back the function of ACL is to have reconstruction of ACL. Even though patient can manage activities like walking climbing stairs, without ACL. ACL reconstruction is indicated in the following:

- To allow return to sports and high level of activity – exercises & jogging
- Those who experience instability.
- Concomitant meniscus and cartilage injury – can give rise to symptoms like locking, pain and joint swellings. When arthroscopy is done for these symptoms ACL reconstruction could also be done at the same time.
- Long term ACL insufficiency can lead to additional meniscus and cartilage damage and result in early wear and tear. Early ACL reconstruction might avoid these complications.

-ACL healing cannot be achieved by any internal medicine or external oil massage. Similar attempts to compensate the ACL deficiency by exercising the muscles have also been futile. The dynamic stability offered by the muscle cannot compensate the static stability afforded by the ACL.

ACL reconstruction is done using few types of grafts and also a variety of fixation methods to fix the graft to the bone. In our center we use all varieties of grafts and techniques. The choice of graft and fixation technique is decided based on the requirements of the patients and the surgeon’s preference. The reconstructed ACL has adequate strength and full return to previous level of activity and sports is possible. The operated patients receive a physiotherapy protocol and specific advice from sports physiotherapist. By improving the muscular strength, flexibility and agility many injuries can be avoided.
